Insured patients can have our services billed to their healthcare provider for Medicare or private insurance reimbursement. Insurance covers unlimited monthly case management if documentation supports patient care. Contact your insurance for coverage details. The following practitioners may bill for CCM services:

●Certified nurse midwives (CNMs)

●Clinical nurse specialists (CNSs)

●Physician assistants (PAs)

  • This plan is tailored for individuals who have Medicare or private insurance. Services are billed to your healthcare provider by ULM Solutions and reimbursed to them through your insurance. You must have 2 or more chronic conditions. This plan allows intense ongoing monthly support to navigate medical records, healthcare coordination, and emergency preparedness with no limits on care time as long as the documentation justifies the care. (Contact your insurance plan to ask about coverage and any additional fees you may be responsible for)

    New Patient Intake Appointment

    • Duration: 60 minutes

    • Format: Telehealth (Virtual video appointment via the SimplePractice app) or In-Person Home Visit

    • Purpose: Establish your healthcare profile, discuss your specific needs, and set up your patient portal.

    Monthly Support

    • Case Manager Access:

      • Call your case manager during normal business hours for general questions, or schedule urgent calls after hours

      • Unlimited scheduled check-in calls from you case manager and Unlimited check-in for updates with all of your healthcare providers

      • Unlimited Telehealth or In Person Home Visits - Choose from 20, 30, or 60 minute visits as many as necessary per month, (No service fee for weekend appointments)

  1. Before signing up, you must review our consent form with your healthcare provider during your next visit. Only the patient will sign the consent form. Fill in the healthcare provider’s information on the 2nd page. E-mail the completed form to intake@ulmsolutions.com or upload it to your portal after you sign up in Step 2. 2. Click the button below to sign up as a new patient. You will receive an email to login to your portal to complete your intake documents when your appointment is approved. We will send your healthcare provider our service agreement to start services when we receive your consent form.

If we don’t receive your signed consent form and service agreement by your first appointment then you will be rescheduled after they are received.
