This plan is tailored for individuals who are slightly complex (2 conditions), semi independent, and need their loved ones and multiple provider coordination with their care. They require regular monthly support in navigating medical records, healthcare coordination, and emergency preparedness.

New Patient Intake Appointment

  • Duration: 60 minutes

  • Format: Telehealth Only (Virtual video appointment via the SimplePractice app)

  • Purpose: Establish your healthcare profile, discuss your specific needs, and set up your patient portal.

    • HIPAA-compliant, secure online platform available 24/7.

    • Complete and manage intake forms.

    • Upload and review all medical records anytime.

    • Unlimited secure text messaging with your case manager.

  • Emergency Contact Access:

    • Two designated emergency contacts will receive access to your patient portal. Can upload files and contact Case Manager

    • Two healthcare providers allowed access to your patient portal. Can upload files and contact Case Manager

  • Medical Records Emergency Storage Device:

    • Choose from a Dog Tag Necklace, Wallet Card, or Bracelet with a USB drive to store your medical records.

    • Ensures access to critical health information if you are unresponsive or without internet access during a medical emergency.

Monthly Support

  • Case Manager Access:

    • Call your case manager during normal business hours for general questions.

    • 1 monthly scheduled check-in call from you case manager and 1 monthly check-in for updates with Two of your healthcare providers

    • Two 20-minute telehealth appointments per month, (Additional 20 minute visit $30 each, $20 service fee for weekend appointments)
