New Patient Intake Appointment

• Duration: 60 minutes

• Format: Telehealth (Virtual video appointment via the SimplePractice app) or In-Person Home Visit

• Purpose: Establish your healthcare profile, discuss your specific needs, and set up your patient portal.

• Emergency Contact Access:

• Medical Records Emergency Storage Device:

  • Choose 2 of the same or different devices: Dog Tag Necklace, Wallet Card, or Bracelet with a USB drive to store your medical records.

  • Ensures access to critical health information if you are unresponsive or without internet access during a medical emergency.

Monthly Support

• Case Manager Access:

  • Call your case manager during normal business hours for general questions

  • 2 monthly scheduled check-in calls from you case manager and 1 monthly check-in for updates with all of your healthcare providers

  • One hour per month of Telehealth or In Person Home Visits - Choose a single 1 hour visit or two 30-minute appointments per month, (No service fee for weekend appointments)
